Privacy Policy

All personal data entered on website will be use only for the purposes of this private practice; appointments, contacting clients, connecting with providers, billing. Data will not be sold or shared in any way.

You will be required to sign The Normal Me HIPAA Privacy Notice which will detail your rights as a patient, the health care provider’s rights, and detail how private health information can be used with and without consent.

Confidentiality

The law protects the relationship between a client and a psychiatric practitioner. Information cannot be disclosed without written permission.

Exceptions include:

Suspected child, vulnerable adult, or elder abuse.

If a client threatens bodily harm to another person/s, a practitioner is required to notify the police and inform the intended victim.

If a client intends self-harm and is not cooperative with measures to insure their immediate safety.


Payment and Cancellation

As you are purchasing a block of time set aside to provide a service dedicated to you, payment is required in advance via Pay Pal or Credit Card. This can be purchased on-line after appointment confirmed.

100% refunds – If cancellation received by required time:
Mon Appts: by noon on Friday.
Tues – Fri Appts: 48 hours prior to appointment.

50% refund – if less than 48 hours.

0% refund – for all no-shows.

If The Normal Me needs to cancel your appointment due to a provider emergency with less than a 24 hour notice, you will receive a 100% refund AND your next visit will be at no cost to you for your inconvenience. The Normal Me reserves the right to change the visit to a tele-psych visit if necessary.

Treatment Policies

The Normal Me reserves the right to make the final decision to accept any client for ongoing treatment after completing the initial Comprehensive Diagnostic Evaluation. Referrals to sub-specialists in the field will be made if it is felt that it would be more appropriate to meet a clients needs. The Normal Me will not accept or provide pharmacotherapy to any client on chronic opiates or actively using substances or alcohol. Substance users will be referred to the well established community resources for assistance in sobriety. The Normal Me reserves the right to discharge any clients who either falsifies information, fails to meet financial obligations, no shows appointments, or abuses medications.

Once accepted as client on Dr. Smith DNP’s panel, the Client-Provider Treatment Agreement form will be signed by both parties. You will be expected to:

  1. Attend scheduled appointments.
  2. Make a reasonable attempt to complete any assigned therapeutic homework.
  3. Obtain prescription refills at the time of the appointment.
  4. Comply with requests for labs deemed necessary for safe treatment.
  5. Address concerns at the time of the scheduled visits. If need arises, request an earlier appointment for follow up.
  6. Utilize the Emergency Room when in crisis or directed to go by Dr. Smith DNP for emergency mental conditions such as suicidal thoughts, homicidal thoughts, uncontrolled mood or psychotic symptoms, or serious reactions to medications.
